Overview
Congratulations on your delegation of permitting, plan review, or inspection responsibilities. For a seamless transition from state to municipal jurisdiction, we have compiled DSPS sample forms and resources utilized in building projects. Also included are some best practices you may find helpful in ensuring consistency and efficiency. These resources are for your use and should be utilized for uniform enforcement of codes for builders and owners doing business statewide.

Plan Review Application
As of April 12, 2021, DSPS utilizes an online submittal platform, eSLA, for plan review submission. Prior to this date, DSPS required the following form to be submitted with the plans, required calculations (structural, building envelope, and heat loss, depending on submittal type), and appropriate review fees. Note that the Supervising Professional’s signature is required prior to submittal for buildings with total volume of > 50,000 cubic feet.
Plan Approval Letter Template
This Sample Plan Approval Letter may be used to correctly identify the project contacts, project location, code requirements, and the limitations of the approval. This letter needs to be attached to the approved plans to complete the “Approved Plans Document” and needs to be kept at the jobsite for inspection purposes.
Plan Additional Information Letter Template
In situations where you do not have complete information or you have identified code violations requiring revisions to the plans, this Sample Additional Information Letter can be used to request additional information or revisions to the plans. This document is sent to the building designer when the building volume will exceed 50,000 cubic feet.

Utilize the following inspection checklists. These provide a complete and comprehensive inspection and documents your findings should you need to provide this information at a later date.
Act 270 Ordinances
2013 Act 270 establishes a uniform commercial building code in Wisconsin while allowing municipalities to grandfather fire detection, prevention, and suppression ordinances that relate to the construction, alteration, or addition to a public building or building that is a place of employment. An ordinance that is grandfathered has passed the requirements laid out by the Legislature in Wisconsin Statute section 101.02(7r)(b).
