Overview

This page contains all information required for Cemetery Associations. Each way to apply has a special subsite denoting specific requirements.

Application Methods

New Licensure

For applicants who have never registered as a Wisconsin Cemetery Association.

Eligibility

Seven or more residents of the same county may form a cemetery association. They shall meet, select a chairperson and secretary, choose a name for the Association, fix the annual meeting date, and elect by ballot not less than three nor more than nine trustees. Immediately following the election, the chairperson and secretary shall divide the trustees by lot into three classes, who shall hold their offices for one-, two-, and three-year terms, respectively.

Within three days of the meeting, the chairperson and secretary shall certify the corporate name of the Association, the names, home addresses and business addresses of the organizers and the trustees, and their classifications, and the annual meeting date acknowledged by them and return the certification to the Cemetery Board. The Association then has the powers of a corporation.

Amendment to a Cemetery Association Certification

A cemetery association may change its name, number of trustees, or the annual meeting date by resolution at an annual meeting, or a special meeting called for such purpose, by a majority vote of the members present.

Submit Form 3169 - Amendment to Cemetery Association Registration and a copy of the resolution to the department.

Annual Reports

All cemetery authorities required to be licensed under this chapter should complete Form 3171 - Cemetery Association Annual Report.

This form may be emailed to DSPSCREDCemetery@wisconsin.gov by the report deadline, or mailed to:

P.O. Box 8935

Predetermination & Legal Information

The term credential is used universally to mean license, certification, permit, registration, enrollment and authorization.

  • Applicants with convictions, who do not already have a credential, may apply for a predetermination. Predetermination is an optional, first step that happens before you would submit an initial credential application while meeting all credentialing requirements (e.g., education, training, examination, etc.)

    • To apply for a predetermination, log into LicensE.

    • If you have not created an account, you will first need to register.

    • After creating your account, log in, and select “Apply for a Pre-Determination.”

    •   Select the type of credential you would like to apply for.

    •   Follow all application instructions and upload all required documents.

  • If the Department decides your record would not disqualify you, you can then apply for the actual credential and meet all the typical requirements like education, training, and exams.

  • You may skip the predetermination process and submit your initial application for your credential. You must complete all steps to meet the credentialing requirements such as education, training, exams, and disclose your conviction record on your application.  Convictions and pending charges will be reviewed as part of the application process.

  • Some applications require review by Department Legal staff.

  • This can be for a number of reasons, including but not limited to disclosure of convictions or pending charges, professional discipline, malpractice, termination, medical condition, supervised experience and application via reciprocity/endorsement.

  • Applications that require legal review will require additional processing time.

  • An application in the status “Legal File Preparation” is with DSPS and under review.
