This page contains information required for the Physician MD and Physician DO credential. Each way to apply has a special subsite denoting specific requirements. Renewal information, forms, and legal information can be found on this page.

Application Methods

Initial Certification

The regular license to practice medicine and surgery for both MDs and DOs.

Renewal After 5 Years

For applicants whose previous WI license expired more than 5 years ago, or who reapply after a licensure lapse due to revocation, voluntary surrender, or a board order requiring reapplication.

Interstate Medical Licensure Compact (IMLC)

For physicians interested in obtaining licensure through the Interstate Medical Licensure Compact (IMLC).

Resident Educational Licensure

For applicants who have been accepted into an accredited post-graduate training program approved by the Medical Examining Board.

Physician Provisional Licensure

For MD applicants who received their education and post-graduate training in a foreign country and have been fully licensed in their country of origin for at least 5 years.

Administrative Physician

Applicants whose primary responsibilities are those of an administrative or academic nature.

Special Permit: Locum Tenens

Apply for a temporary permit to work as a Locum Tenens physician. Compare to Bridge Temporary Credential.

Special Permit: Visiting Physician

Apply for a temporary permit to work as a Visiting Physician.

Special Permit: Camp

Apply for a temporary permit to work at a camp or other recreational facility. Compare to Temporary Practice at Camps (below).

Military Reciprocity

Service members, former service members, and spouses credentialed in another jurisdiction may apply for a reciprocal credential.

Bridge Temporary Credential

This temporary license is for health care providers who seek to immediately begin practicing in Wisconsin while their application for a permanent license is under review. Compare to Special Permit: Locum Tenens.

Credentialing Timeline: Please Plan Ahead

Applicants, recruiters, institutions, and others involved in the placement of individuals who seek to be credentialed in the state of Wisconsin should understand that the credentialing process takes time and that credentialing is not guaranteed to any applicant. Factors that determine the length of time it may take to process an application include the length of time the applicant has been in practice, the total number of jurisdictions in which the applicant has been credentialed, and the length of time it takes for supporting documents to be received in the Board office and reviewed.

The application consists of an all-inclusive packet with instructions and information on all applicable requirements. We strive to process applications in a timely fashion. We cannot issue a credential until all of the required documents have been received and reviewed in the Board office.

It is the Department’s mission and legislative mandate to provide consumer protection for Wisconsin residents. The Department and the Board have been asked to waive requirements to expedite the process, only to discover legitimate grounds to deny a credential. This can present a serious problem for the applicant, recruiter, or institutions if the applicant has relocated, purchased property, or made other commitments prior to the issuance of a Wisconsin credential. We urge you not to make these moves until you know that your credential has been issued. Please “plan ahead” as we cannot speed up the credentialing process or waive supporting documents even in emergency situations.

Temporary Practice at Camps

Physicians licensed to practice medicine and surgery in another state or territory of the United States or a Canadian province or territory may practice medicine and surgery to provide treatment to campers and staff for not more than 90 days in any year without holding a Wisconsin license.

Complete Form 5568 Out of State Physician Temporary Camp Practice Notification Form, and email directly to DSPSCredMedBd@wisconsin.gov. Include the word “camp” in the subject line.

Submit any follow-up questions via a support ticket on LicensE. Support tickets are the most expedient way to request support. Email inboxes are not monitored for support purposes.

Form 5568 must be submitted prior to practice.

Sports Physician Licensure Exemption Extension

If you require an extension of the ten-day exemption to practice without a Wisconsin license as a sports physician for a sports team or national sports governing body while in Wisconsin, complete and return Form 3220, Application for Extension of the Sports Physician Licensure Exemption.

To Renew:

You will be able to renew your credential approximately 4 weeks prior to your license expiring. You will receive an email to the email address on file with instructions on how to renew.

  1. ​Online Log In: Go to LicensE

  2. Select "Renew My License"

  3. Complete the application

  4. Upload any required documents

  5. Submit payment

If you hold a Physician License obtained through the IMLC, you must renew online through the IMLC (link below). Only licenses obtained through the IMLC may renew through the IMLC site.​

REL renewals are completed through our LicensE system. To complete an REL renewal application, log into your LicensE account and select Renew My License. You may also access the REL renewal application by selecting My Approved Licenses and Renew from the dropdown arrow under Action. If you are interested in converting your regular physician license to an administrative license, or vice versa, complete and return Form 3074, License Transition Addendum.

  • To complete the renewal process, you will be asked to complete an optional survey that is given by the Department of Workforce Development (DWD).

30 hours of Category 1 AMA or AOA is required

For the 2027 renewal, each license holder will be required to take two of the required 30 hours related to prescribing opioids and other controlled substances. These courses must be AMA or AOA Category 1, but no longer require specific approval from the Medical Examining Board. Physicians who do not hold a U.S. Drug Enforcement Administration number to prescribe controlled substances are exempted under the rules.

This is separate from the 8 hour requirement from the DEA. License holders should contact the DEA regarding those hours. Courses taken for the DEA requirement can be used for Wisconsin renewal as long as they meet the requirements in Med 13 of the Wisconsin Administrative Code.

The Medical Examining Board Partners with CE Broker

Payment Information

  • You can pay all fees associated with your credential by going to LicensE. 

  • You will be asked to pay automatically at the end of your application. 

  • You can pay with a credit or debit card, or via an automated clearing house or (ACH) payment.  

Please note, using an ACH payment can increase credential processing times by 3-5 business days.

Predetermination & Legal Information

The term credential is used universally to mean license, certification, permit, registration, enrollment and authorization.

  • Applicants with convictions, who do not already have a credential, may apply for a predetermination. Predetermination is an optional, first step that happens before you would submit an initial credential application while meeting all credentialing requirements (e.g., education, training, examination, etc.)

    • To apply for a predetermination, log into LicensE.

    • If you have not created an account, you will first need to register.

    • After creating your account, log in, and select “Apply for a Pre-Determination.”

    •   Select the type of credential you would like to apply for.

    •   Follow all application instructions and upload all required documents.

  • If the Department decides your record would not disqualify you, you can then apply for the actual credential and meet all the typical requirements like education, training, and exams.

  • You may skip the predetermination process and submit your initial application for your credential. You must complete all steps to meet the credentialing requirements such as education, training, exams, and disclose your conviction record on your application.  Convictions and pending charges will be reviewed as part of the application process.

  • Some applications require review by Department Legal staff.   

  • This can be for a number of reasons, including but not limited to disclosure of convictions or pending charges, professional discipline, malpractice, termination, medical condition, supervised experience and application via reciprocity/endorsement.   

  • Applications that require legal review will require additional processing time.  

  • An application in the status “Legal File Preparation” is with DSPS and under review.

LicensE

  • LicensE at (license.wi.gov) is a self-guided online system where you can manage your professional credential from start to finish. 

  • You will still need to review the requirements for the credential you are applying to. These can be found on the A-Z profession page. 

  • With LicensE you can: 

    • Apply for a credential for the first time, with steps guiding you to upload your application documents, and pay all required fees.  

    • Renew an existing credential when it is time, to keep your credential active.  

    • View or download your credential for your records or verification.  

    • Update your personal information, including your phone number, email, and mailing address 

    • Submit support tickets and receive responses to your email from our staff experts.  

    • Allow third parties to review and upload required documents on your behalf 

    • Check the status of someone's credential using the public LicensE look up. 

  • If you have never held a credential, you will need to create an account the first time you are on LicensE. 

    • To create an account, visit LicensE. You will then select “register for an individual” if you are applying for a license held by an individual, or “register for an organization” if you are applying for a license held by an organization.
